Risk Management in the NAPT customer portal is where a laboratory records risk, keeps a history of what was decided, and saves a PDF. Open Risk in the portal to use it.
There are two assessments. They can be submitted on different schedules. Submitting one does not change the score, the saved results, or the 30-day reminder of the other.
Lab Assessment
Lab Assessment scores the laboratory as a whole. It starts with ten risk categories. Each category is scored, notes can be stored with the score, and the categories combine into one weighted score. By default each category counts for 10%. Account administrators can change those weights. The weights must total 100%.
The ten starting categories are Organizational Context, Scope of Accreditation, Workload, Staff Capabilities, Laboratory Resources, Changes in Standards, Equipment, and Methods, Proficiency Tests: Past Failures, Audits: Technical Findings, Technician Training, and Certificate or Report Errors. An administrator can rename a category, describe it, or leave it out of the assessment.
When the scores are ready, Review & Submit saves a snapshot. You can add a summary in your own words or draft one with Generate Summary via QMS AI and edit it. If a lab snapshot was saved in the last 30 days, the portal asks you to confirm before saving another one. Earlier snapshots stay available from View Saved Results, and each one can be saved as a PDF. The dashboard Risk panel shows the latest lab snapshot.

PT Risk
PT Risk scores the disciplines and parameters already on the PT Plan. The list is filled from that plan. You record notes and choose an overall risk from the same scale labels used on Lab Assessment. An account administrator turns this tab on. Until then, only Lab Assessment is shown.
Review & Submit on this tab saves a separate snapshot of the plan at that moment, including parameters that have not been assessed yet. It has its own saved results, its own 30-day reminder, and its own Save As PDF. Those results are not added to the lab weighted score.

Shared Settings
The two assessments share Framework Settings and nothing else. An account administrator sets the score range, whether a higher number means lower risk, the wording of each score, which lab categories appear, and whether PT Risk is turned on. Other users can open View Settings to see the factor weights. Administrators change those weights with Factor Weights.
