PT Risk records risk for the disciplines and parameters already on your PT Plan. The list is filled from that plan. You do not type the discipline or parameter by hand.

Turn On PT Risk

  1. Open Risk in the NAPT customer portal.
  2. On Lab Assessment, click Framework Settings. This button is available to account administrators.
  3. On the Scale & Scoring tab, select Enable parameter/discipline operational risk.
  4. Click Save Settings.

A PT Risk tab then appears next to Lab Assessment. See Framework Settings for the rest of that screen.

Review Plan Parameters

Open PT Risk. Each row is one discipline and parameter from your PT Plan. If the same parameter is scheduled more than once, it appears once, and the Latest on Plan column shows the most recent scheduled entry.

If the list is empty, the PT Plan has no parameters yet. Add them on the PT Plan page, then return to PT Risk.

Assess a Parameter

Click a row. Discipline and Parameter are already filled from the PT Plan and cannot be edited on this screen.

  1. Enter Risk Notes. Notes are required.
  2. Choose Overall Risk from your scale labels, or click Auto-Suggest.
  3. Click Confirm to save the row. Click Close to leave without saving.

Auto-Suggest

Auto-Suggest uses the discipline and parameter already selected, including the matching PT Plan entries and recent unsatisfactory results. It can fill the overall risk score and a short rationale in Risk Notes. Edit both before you click Confirm. Auto-Suggest does not save the row.

Confirm saves that parameter on the PT Risk tab. It does not create a snapshot, and it does not change the Lab Assessment score.

Review and Submit

Lab Assessment and PT Risk are submitted on their own schedules. They share Framework Settings only: the scale, the direction, the score labels, and the switch that turns PT Risk on. A PT Risk snapshot does not change the lab weighted score, the lab saved results, or the lab 30-day reminder.

When the PT Risk list is what you want to keep, click Review & Submit. This button stays unavailable until the PT Plan has at least one parameter. The snapshot includes every parameter on the plan at that moment. Confirmed rows keep the score and notes. Parameters you have not assessed are included and marked Not assessed.

The review screen shows who is submitting, the date, and a table of discipline, parameter, overall risk, status, and whether notes were entered. Click Save.

  • If a PT Risk snapshot was submitted within the last 30 days, choose Submit Anyway to add another snapshot, or Go Back to keep the earlier one. This reminder looks only at PT Risk snapshots.
  • Choose Yes – Enter Summary to write a summary, or No – Save Results to store the snapshot without one.
  • In the summary box, type the text or click Generate Summary via QMS AI. You can edit the draft. Then click Save Summary and Results. Close and Do Not Save leaves without storing the snapshot.

Saved Results and PDF

Click View Saved Results to open earlier PT Risk snapshots. The list shows the snapshot date, who completed it, how many parameters were included, and how many were assessed. These rows are separate from Lab Assessment results.

Open a row with View Details. Choose Save As PDF to download the report, or Delete Snapshot to remove it. Deleting a snapshot cannot be undone. It does not change the working scores on the PT Risk tab, and it does not delete a lab assessment.
